Dear Chesapeake HS Athletes and Families: 

Fall Sport students must have maintained a 2.0 GPA during the fourth quarter marking period(excluding Grade 9*). Final grades are not used to determine academic eligibility. Attending summer school to adjust the GPA is not permitted. BCPS Rule 6702/Athletic Eligibility

*This regulation does not apply to incoming 9th grade students for fall eligibility (1st quarter grading period).  


Forms Required Before the 1st Day of Practice/Try-Outs - No Exceptions!

If you need assistance with sports equipment and/or gear, please contact the coach.

Double-check your Student’s Fourth Quarter Report Card (2.0 GPA required during fourth quarter). No need to register on Form Releaf if they are ineligible; the student will not be allowed to play.

Pre-Participation Physical Examination Form completed by a doctor or nurse practitioner.

Form Releaf Online Registration 

Before registering be sure to have doctor information and health insurance information handy. You can log back into Form Releaf website to update your information in “Registration History.” Contact customer service for support at 844-367-6735.

Signup for your Form Releaf account by entering your namee-mail address and password.

You will receive confirmation from Form Releaf once required form is completed.

Physical Exams required to be given to coach prior to practicing.

Mr. Downey, Athletic Director will receive an email confirmation that your registration was successful.
